Appointment Information

An MCS staff member is available to answer your call in person between the hours of 10:00 am and 2:00 pm on Tuesdays and Wednesdays.  Call 404-321-1794 for more information or to schedule an appointment.

If you call during other times, also dial 404-321-1794.  You can leave a message on our Intake and Information Line Voicemail at Ext. 1.  Be sure to include your name, phone number(s), and best times to reach you.  A therapist will return your call promptly, answer your questions, and help you schedule an appointment.

Once you have scheduled an intake appointment with us, please download a copy of our New Client Information form.  If you have access to a computer printer, please print a copy of this form, fill it out, and bring it with you to your intake appointment.  If you are unable to fill out the form in advance, you may fill it out at our office immediately before your intake appointment is scheduled.
